• Sworn Statement Format.
• Payment certificate for Rights of Procedures of S/. 262.40.
• A single photocopy of your currently RUC
• A DNI or LE photocopy of the legal representative of the Company. (In case of a corporative body).
• Photocopy of the company Constitution registered in the public record book.
• Photocopy of the present application (Literal Form).
• A Guarantee Letter in favor of ZOFRATACNA, equivalent to 10 UIT, in any of the following ways:
Cash
Guarantee Letter
Bond Policy
Property mortage outside of Zofratacna
• Certificate of work approval.
• Description of :
Computer services
Security equipment
Manipulation Equipment
• Two (02) passport photos
• Lots cession contract (only if the lot is been demised, other way you should give an Authorization form from the General Management Office to rent the lot and should give a photocopy of the Rent Contract).

• An unconditional, irrevocable, charitable, equivalent to 10 UIT, guarantee letter in favor of ZOFRATACNA, automatically attainable in any of the following ways:
Cash
Guarantee letter
Bank Certificate
Property mortgage out of ZOFRATACNA.
• Computer services
Security equipment
Merchandise manipulation
• Two (02) passport photos from the user or legal representative.
